All couples who have not achieved pregnancy naturally within 6 months to a year are advised to undergo investigation of the possible cause.

In one day, without pain and anaesthesia you can get a diagnosis of couple sterility in the hospital “Genesis” This process includes:

When coming to consultation, couples should bring:

  • hormones’ analysis on the third day of the cycle: FSH, LH, Estradiol, AMH, sTHS, ATPO, Prolactin,
  • Smears: vaginal discharge, Papanicolaou, cervical smear, Ureaplasma, Mycoplasma, Chlamydia tr (the last three smears should be taken from both partners),
  • Complete blood count, erythrocyte sedimentation rate, urine analysis
  • Insulin resistance

Step before IVF

The basic prerequisite for the successful treatment of sterility is a quality diagnosis. It provides insights into the cause and accurately determines the selection of appropriate method. Every detail in this way can be important because it can show that in vitro fertilization may not be necessary and that some other methods may lead to the desired result.

ERA test
Era test

It determines genetic endometrial receptivity, that is, the personalised window of implantation of each woman. The test showed that 27% of patients are not receptive according to the protocols which include implantation of the embryo on the third or the fifth day after taking oocytes and fertilization.

According to the clinical analysis, ERA test success rate is more than 25 % for patients of all ages who have had one or more unsuccessful IFV procedures and have problems with implantation of a fertilized egg in the uterus.
If you do a combination of ERA and PGS testing, the success rate is 80%.

The most renowned genetic laboratory “Igenomix” from Spain has chosen hospital “Genesis” from Novi Sad to be the first in Southeast Europe to introduce this most advanced non-invasive prenatal genetic test.
The test is carried out by doing a endometrium analysis, and the results are obtained within about 2 to 3 weeks.

SATCTG dijagnostika

It is a diagnostic test that should determine whether there are genetic causes of male infertility. It enables the analysis of chromosomes 13, 18, 21, X and XY because they are usually responsible for spontaneous abortion, and lead to transmission of chromosomal abnormalities to the embryo.

This test identifies men with low success in reproduction and provides useful information for personalized genetic counselling of couples before starting IVF treatment. They are generally advised to undergo PGS test, which allows the selection of chromosomally normal embryos, increases the effectiveness of IVF and chances of getting healthy babies.

The test results can be expected two weeks from giving the blood sample.

This test estimates the genetic compatibility before planned pregnancy in order to determine whether the couple carries the risk of developing any genetic anomalies. If both partners are carriers of mutations in the same gene, the test will help prevent giving birth to children with genetic disorders.

CGT test helps to detect 15.000 mutations and 600 genetic disorders, such as cystic fibrosis, muscular dystrophy, Gaucher’s disease, anaemia, and other phenomena.

If the result is positive for both partners, it is very likely that they will have a sick child. They might have healthy children if they do Preimplantation Genetic Diagnosis – PGD. There is also the possibility of natural conception with proper prenatal diagnosis.

30 to 40 days after taking the sample are necessary for the analysis and the preparation results.

Semen analysis

It represents a laboratory analysis of sperm which includes both macroscopic and microscopic examination. Macroscopic examination refers to the volume of sperm, its look, liquefaction, viscosity and pH value.
Microscopic examination includes the number of spermatozoa, their motility, the number of present leukocytes, erythrocytes, epithelial cells, agglutination of the spermatozoa, determination of the vitality of normal and abnormal spermatozoa and the presence of fructose in the sperm.

Normospermia – normal sperm count, represent a semen analysis result in which the number of spermatozoa is greater than 20 million / ml, their progressive motility is greater than 50% and the number of normal forms is greater than 50%.

More detailed examinations are related to the number and morphology of the sperm cells, the presence of pathological forms, determination of their vitality, i.e. the motility and so on.

Ultrasound examinations

3D and 4D ultrasound examination give extremely important information during the process. These are non-invasive and safe examinations that provide three-dimensional image (3D) and three-dimensional film (4D). They significantly facilitate the analysis because of their transparency and clarity, and these examinations are done so as to gain insight into the size, composition and structure of organs, as well as the existence of possible pathological structures in the pelvis.

In addition to ultrasound examination of pelvis, it is also recommended to do the examination of the thyroid gland and breasts during the diagnostic process.

Increased or decreased release of thyroid hormones has influence on the heart, metabolism, nervous system, and the female reproductive system. Therefore, information on the hormonal status, as well as the existence of possible neck lumps and masses is very important in the process of finding the cause.

An ultrasound examination is done together with the analysis of blood, and then, if necessary, the endocrinologist determines the required medicine and the dose, in order to create conditions for the smooth implementation of the IVF process.

Breast examination and BRCA ½ test

Family history of breast cancer is very important information in determining the stimulation scheme in the IVF process. Consequently, these examinations and tests are very useful to do.

Genetic screening test BRCA 1 and BRCA 2 are carried out by a technique of sequencing which aims to locate mutations in both genes. They are responsible for 5 to 10% of all breast cancers, and for 10 to 15% of all ovarian cancers.

If a woman has inherited the BRCA 1 and BRCA 2 gene mutation from her parents, she is facing an increased risk of developing breast cancer and ovarian cancer. A positive test result has important consequences on the health of family members, and even on future generations, because the person who inherits harmful mutations in these genes, can transfer them to sons and daughters. Every child in this case, has a 50% chance of inheriting the mutation from their parents, if the parent is the mutation carrier.

Testing is done by analysing blood samples in the usual way, and the results are ready in a month.

Tubal patency

In order to check tubal patency we use a new, painless method, controlled by ultrasound examination with special gel at hospital “Genesis”. It is a sonohysterosalpingography. This examination takes only 8 minutes.
The examination is done before ovulation and the patient needs to have vaginal and cervical smears with normal results.

Hysteroscopy

It is a one-day mini-invasive procedure that significantly affects the improvement of the overall results of IVF in indicated cases. The importance of this method lies in the fact that it includes direct examination of the uterus in which the future embryo should develop.

During this intervention adhesions which may have been result of the earlier interventions in the uterus can be also removed, and congenital and acquired diseases of the uterine cavity are also treated: uterus septus, polyps and smaller fibroids. It is done with a short-term anaesthesia and an hour after the intervention, patients are able to go home.

Diagnostic puncture and biopsy of the testicles

This surgery is used to extract sperm cells from the testicular tissue in cases in which there are no sperm cells in the ejaculate (azoospermia). The method is painful, so we carry it out with a short-term general intravenous anaesthesia. After testicular puncture and the selection of testicular sperm, fertilization, i.e. insemination of oocytes, is carried out by the ICSI method.

Laparoscopy

Diagnostic laparoscopy is an important method of assessment of reproductive organs of women. It examines the state of the fallopian tubes and detects genital system anomalies, causes of a variety of painful conditions in the pelvis, inflammation, endometriosis and other disorders.

It is an endoscopic, minimally invasive surgery which has become dominant during the majority of gynaecological surgeries. It solves the problem of a number of different types of ovarian cysts, fibroids, and other potential causes of infertility in some of the stages of the treatment.

Success is the same as in the operations carried out in the traditional manner, but when compared to the traditional surgery, there are many advantages. Recovery is faster and less painful. The average hospital stay is one day.
